Qualifications

• BSN and Illinois RN license are required.

Responsibilities

• Assess and records patient status and care needs upon admission.
• Organizes the assessment health data for accuracy and accessibility.
• Identifies problems that need immediate attention.
• Document diagnoses and plan of care to support outcomes.
• Initiates discharge planning and coordinates with patient/family and health care team.
• Provide patient and family education based on evidence-based practice.
