Job summary

Boston Medical Center is seeking a Social Worker MSW/LCSW to join the Care Management team. The role provides direct counseling, psychosocial assessments, and referrals across units, with an emphasis on crisis intervention and family support.

The position includes supervision of graduate students and collaboration with an interdisciplinary team to plan ongoing discharge needs and community referrals, ensuring compassionate care throughout hospitalization.

Qualifications

• Master’s Degree in Social Work required.
• Experience working with patients and families in crisis preferred.

Responsibilities

• Provide individual and/or group counseling to patients and families.
• Supervise graduate social work students when indicated.
• Perform diagnostic evaluations and psychosocial assessments.
• Provide referrals and follow-up services.
• Collaborate with interdisciplinary team for continuing care plans.
• Offer crisis intervention when needed.
• Maintain timely social work documentation in patient records.
• Lead or co-lead a patient/family support group when appropriate.
