Job summary

Qualifications

Responsibilities

• Assesses the psychosocial status of patients and communicates findings to the care team.
• Carries out social evaluations including family dynamics, caregiver abilities, and plans intervention.
• Counsels patient and family/caregivers as needed in relation to stress and coping difficulties.
• Maintains clinical records on all patients referred to social work and documents at Point of Service.
• Provides information and referral services for patients and families regarding practical and environmental needs.
• Serves as liaison between patients/families and community agencies.
• Participates in discharge planning when needed.
