Job summary

Intergrated Pain Management SC is seeking Emergency Department Discharge Planners to join a local hospital ER as a third‑party partner. Night, overnight, holiday and weekend shifts are required, with weekend rotations encouraged.

In this role you will identify eligible accident patients in the ER, initiate contact after stabilization, and coordinate follow‑up care with hospital teams. You will document interactions in EMR, provide resources and legal referrals if requested, and ensure patient

Qualifications

Healthcare knowledge in ER settings and follow-up care coordination.
Experience with EMR or patient documentation systems.
Ability to coordinate with ER staff and external case management teams.
Familiarity with insurance processes and legal resources for accident-related care.

Responsibilities

Identify eligible accident patients in the ER, initiate contact post-stabilization, and provide an overview of follow-up care services.
Collaborate with the hospital care team to ensure smooth handovers of patient information for ongoing external care coordination.
Provide patients with resources and information on next steps and legal assistance if requested.
Accurately document all interactions within the system, ensuring data privacy and compliance.
