AtlantiCare seeks an Emergency Department Case Manager to assess level of care and coordinate discharge planning with the clinical team. The role aims to promote patient wellness, improve outcomes, and ensure efficient use of health services.

The position requires RN licensure in New Jersey, BLS/HCP certification, and prior case management or insurance case management experience is preferred. A BSN is preferred and clinical applications proficiency is expected.

Qualifications

• Graduate of an accredited school of nursing required.
• Bachelor's degree in Nursing preferred.
• Case Management certification preferred.

Responsibilities

• The Emergency Department Case Manager provides level of care assessment and discharge planning in collaboration with the clinical team and medical provider.
• Promotes patient wellness and improved care outcomes, efficient utilization of health services, and minimizes denials of payment.
• Supports organizational goals by providing quality customer service and participating in performance improvement efforts.
