Job summary

Wellstar Health System seeks a Nurse Navigator to support patients and families through the cancer care continuum. The role advances access to care and ensures care plans align with evidence-based practices, patient needs, and program goals.

The Nurse Navigator will collaborate with clinicians and administrators to remove barriers, provide education, and coordinate multidisciplinary care across the Wellstar network.

Qualifications

• RN with a state license and required certifications on hire.
• Minimum 5 years nursing experience with 2 years in disease-specific care.
• Outpatient care or care coordination experience preferred.

Responsibilities

• Serve as subject matter expert in patient navigation.
• Improve patient access to timely care.
• Collaborate with program leaders to assess community needs.
• Coordinate care across specialty clinics and multidisciplinary rounds.
• Provide education and resources to patients and families.
