Job summary

The Nurse Navigator (NN) at Wellstar Health System is a licensed registered nurse with disease-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome barriers to care and ensure timely access to quality health and psychosocial care throughout all phases of the cancer continuum.

This role is patient- and provider-facing, collaborating with program leaders and the navigation team to coordinate care, document thoroughly, and support

Qualifications

• Minimum 5 years of nursing practice.
• At least 2 years in disease-specific care.
• Outpatient care or care coordination experience preferred.
• Must achieve disease-specific certification within 1 year from start date.

Responsibilities

• Serves as subject matter expert in the field of navigation.
• Improves patient care timeliness and access to care.
• Collaborates with program leaders and administration to evaluate data and identify areas of improvement.
• Performs and coordinates care with diverse interdisciplinary teams.
• Completes documentation and promotes a respectful, inclusive clinical environment.
• Coordinates care across specialty clinics and multidisciplinary rounds.
