Job summary

Health Choice Network, Inc. is seeking an Application Analyst, Value-Based Services (Healthy Planet) to implement, configure, maintain, support, and optimize Epic Healthy Planet applications and related workflows.

This role partners with clinical and operational staff to advance population health initiatives and value-based care programs. Ideal candidates will have strong Epic Healthy Planet knowledge, experience with ACO operations, population health initiatives, and data-driven reporting.

Qualifications

Experience with Epic Healthy Planet and value-based care workflows.
Experience supporting ACO operations and population health programs.
Experience with CMS, HEDIS, and PCMH reporting.
Experience with data mapping, file imports, and system configuration.
Ability to translate business requirements into technical solutions.

Responsibilities

Serve as primary application support contact for Epic Healthy Planet users.
Configure and optimize Healthy Planet modules including data marts and dashboards.
Support ACO operations and population health initiatives.
Analyze and resolve system and workflow issues affecting reporting.
Collaborate with cross-functional teams to improve processes.
Develop system documentation and coordinate testing activities.
