Job summary

Prestige Healthcare Resources, Inc. is seeking an EPD Waiver Case Manager in Washington, DC to ensure Medicaid Waiver beneficiaries receive coordinated, person-centered services.

You will maintain records, conduct assessments, develop service plans, and monitor progress while navigating public benefits and collaborating with providers. Caseload management up to 45 clients and on-call responsibilities are included.

Qualifications

• One of the listed licensure and experience combinations is required.
• Strong knowledge of case management principles and HCBS Medicaid Waiver familiarity preferred.
• Excellent written, verbal, time management, and organizational skills.
• Proficiency in web-based documentation systems and Microsoft Office 365.

Responsibilities

• Maintain and update beneficiary electronic records with HIPAA compliance.
• Conduct intake, annual recertification, and ongoing assessments using person-centered planning.
• Develop PCSPs reflecting beneficiaries’ goals, preferences, resources, and needs.
• Provide ongoing case management and monthly monitoring with home visits.
• Complete quarterly PCSP reviews, including risk assessments and service utilization monitoring.
• Advocate for public benefits and connect beneficiaries to resources.
• Manage caseload up to 45 beneficiaries.
• Respond to crises and participate in on-call coverage.
