Qualifications

• Washington RAAC credential required or eligible to obtain.
• Background check per WA state; may require national check if not consecutively in WA in last 3 years.
• Ability to communicate effectively verbally and in writing.
• Ability to work as part of a multi-disciplinary team.

Responsibilities

• Educates individuals on consents for treatment and authorizations.
• Participates in interdisciplinary treatment team meetings.
• Coordinates external appointments and discharge planning for patients; ensures resources are aligned with both facility staff and external providers.
• Identifies and addresses gaps in service needs for participating parties and makes recommendations at discharge.
• Maintains accurate clinic records and documents all services and activities in the medical record.
