Job summary

Amedisys is seeking a registered nurse to join our hospice team in Alabama. This role involves collaborating with the interdisciplinary team to apply patient-specific plans of care, coordinate assessments, and manage clinical episodes to ensure quality end-of-life care.

The RN will educate patients and families, participate in care planning, and support staff through competency validation and on-call duties.

Qualifications

• RN license required in state(s) of assignment.
• Current CPR certification.
• Less than one year of clinical RN experience.
• Approval by VP of Clinical.
• Strong communication with diverse age groups and cognitive abilities.
• Knowledge of Federal/State rules, ACHC standards, and regulatory requirements.
• Valid Driver's License and reliable transportation.

Responsibilities

• Provide hospice nursing care per patient plan of care and policies.
• Perform comprehensive patient assessments and collaborate with IDT to develop plans.
• Manage clinical episodes and care plan changes per status or physician orders.
• Deliver high quality pain and symptom management and end-of-life care.
• Coordinate with care team to meet patient needs and referrals as needed.
• Initiate hospice and palliative care interventions as appropriate.
• Support patient/family emotionally and participate in care planning.
