Zusammenfassung

Amedisys is seeking a Registered Nurse to join our hospice care team in Tennessee. You will collaborate with the HSP Clinical Manager and IDT to assess patients, develop and modify care plans, and ensure high-quality end-of-life care with effective pain management.

You will supervise a caseload of LPNs and Hospice Aides, coordinate with PRN staff, and document care activities with adherence to policy and regulatory standards. A strong focus on teaching patients and families is required.

Qualifikationen

• Current, unencumbered RN license for the state(s) assigned.
• Current CPR certification.
• Less than one year of clinical RN experience.
• Must be approved by VP of Clinical.
• Ability to communicate with individuals of all ages.
• Demonstrates strong written and oral communication skills.
• Knowledge of Federal/State rules, ACHC standards, and regulatory requirements.
• Valid Driver's License with reliable transportation and insurance.
• Basic computer skills and familiarity with online HR tools.

Aufgaben

• Collaborates with the HSP Clinical Manager and Peer Mentor to provide hospice nursing services and case management.
• Performs comprehensive patient assessments with IDT to develop the individualized plan of care.
• Manages clinical episodes and documents plan of care changes as needed.
• Delivers high-quality end-of-life care with pain and symptom management.
• Works with family/caregivers and interdisciplinary team to meet patient needs.
• Initiates hospice/palliative care interventions as appropriate.
• Provides patient/family teaching on pain and symptom management.
• Supervises caseload of LPNs and Hospice Aides; coordinates care with PRN staff.
• Documents admissions, recertifications, deaths, ABNs, and physician orders timely.
• Maintains ongoing communication with the Clinical Manager on patient progress.
• Participates in care center performance improvement initiatives.
