Job summary

Mental Health Resource Center in Jacksonville seeks a Community Resource Specialist to help adults access entitlement programs (Medicaid, SNAP, Social Security, housing) and community resources at the South Comprehensive Services Center. The role follows ROSC principles and person-centered approaches.

The specialist will assist with resource identification, applications, document gathering, and navigation of the ACCESS system, working with SOC Navigators and the CSC team.

Qualifications

• Requires high school diploma and at least six months of experience in human services, healthcare, social services, or a behavioral health setting.

Responsibilities

• Assesses entitlement and community resource needs for walk-ins or SOC referrals.
• Provides direct assistance with resource identification and entitlement applications.
• Helps individuals navigate the State ACCESS system and gather documents for applications.
• Refer eligible individuals to SOAR processors for Social Security assistance.
• Tracks pending applications and reports outcomes to the SOC Navigator.
• Refers walk-ins needing behavioral health services to the SOC Navigator and shares outcomes.
• Maintains and updates the Community Resources folder with housing, childcare, food banks, and recovery supports.
• Develops a monthly calendar of free/low-cost events in the Jacksonville area.
• Communicates additional service needs to the CSC team and SOC Navigator.
• Provides back-up support for CSU discharge linkage when assigned.
• Maintains confidential documentation in EHR and tracking tools.
