Qualifications

• Bachelor's degree in one of the following fields: Social Work, Psychology, Sociology, Counseling, Human Services, Behavioral Health or related field.
• No prior case management experience required; comprehensive training provided.
• Bilingual English/Spanish applicants encouraged to apply.

Responsibilities

• Provide psychosocial rehabilitation and case management services per Medicaid guidelines.
• Coordinate, implement, and monitor individualized treatment plans.
• Maintain an assigned caseload with person-centered services.
• Complete assessments, treatment plans, progress notes, and required documentation.
• Ensure services comply with Medicaid, Medicare, and payer requirements.
• Collaborate with medical providers, therapists, intake staff, and other team members.
• Participate in supervision, staff meetings, trainings, and quality improvement activities.
• Respond to client needs and crises during scheduled hours.
