Job summary

STEP, Inc. is seeking an entry‑level care manager to deliver assessment and care planning for older adults, helping them stay independent and connected in the community.

The role develops plans from evaluating social supports, daily living activities, cognition, physical health and resources, with some independent judgment while adhering to established practices.

Travel may be required, valid driver’s license and reliable transportation are needed to fulfill agency duties.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• One year of public or private social work experience with functional/psychosocial assessments preferred.
• Proficiency with MS Office (Word, Access, Excel) expected.

Responsibilities

• Provide assessment and care management services to Older Adults with functional disabilities.
• Develop care plans based on evaluation of social contacts, ADLs, cognition and health.
• Follow established procedures; exercise independent judgment when needed.
