Qualifications

• Master’s degree in Nursing, Healthcare Administration, Business Administration, Public Health, or related healthcare field.
• Active RN license or LICSW; RN licensure preferred where required.
• Ten years of progressive healthcare leadership experience.
• Five years of senior leadership experience in care management, population health, or value-based care.
• Demonstrated experience leading enterprise-wide clinical or care continuum transformation.
• Ability to lead across matrixed environments with physician, nursing, operational, finance, and external partners.
• Strong financial acumen, strategic planning, regulatory understanding, and performance improvement expertise.
• Excellent communication and executive presence with stakeholder engagement.

Responsibilities

• Provide executive leadership for enterprise care management continuum across Care Management functions.
• Design and implement an integrated enterprise operating model for care management.
• Establish system-wide standards, governance, workflows, and performance metrics for care management.
• Lead strategies to improve patient progression, reduce avoidable utilization, and optimize length of stay.
• Collaborate with leaders to align care management with quality, access, and financial goals.
• Oversee workforce planning, leadership development, and performance expectations for care management teams.
• Provide budget accountability and resource planning for care management programs.
• Utilize data and analytics to monitor performance and drive continuous improvement.
