Job summary

Enrollment Specialist at Trinity Health PACE Broad Street plays a key role in guiding prospective participants through the enrollment process, from referral to eligibility assessment. Qualifications

• Associate’s degree with 2+ years in sales or marketing preferred (or equivalent experience).
• Valid driver’s license, auto insurance, and reliable transportation required.
• Strong communication, interpersonal, and organizational skills.
• Proficiency in Microsoft Office and experience with CRM/EHR platforms.
• Ability to work independently and adapt to a dynamic work environment.

Responsibilities

• Serve as the first point of contact for new referrals.
• Educate prospective enrollees and families about the PACE program.
• Gather and input clinical, financial, and demographic data into CRM and EHRs.
• Coordinate all stages of enrollment, including scheduling assessments and Medicaid applications.
• Develop and maintain relationships with referral sources, families, and community partners.
• Represent the organization at community outreach events, health fairs, and sessions.
