Enrollment Specialist at Trinity Health PACE Broad Street plays a key role in guiding prospective participants through the enrollment process, from referral to eligibility assessment. Qualifications

This position supports the entire enrollment process, from initial referral to eligibility assessment, ensuring accurate documentation and seamless communication between participants and the interdisciplinary team.
