Senior Community Care of Kentucky PACE, part of Volunteers of America, seeks an Enrollment Specialist in Louisville, KY. The role focuses on admissions coordination, pre-admissions counseling, and referrals to community resources.

You will promote the VOAN SCK PACE program across sites and in the community, supporting a mission-driven, patient-centered environment. Qualified candidates have a Bachelor's degree (humanities/health and human services) and CNA/LPN certification, plus 2 years in

• Bachelor's degree in humanities/health and human services or equivalent.
• Two years of marketing experience with emphasis on sales/admissions in health care.
• CNA or LPN certification.

Responsibilities

• Process and complete admissions and pre-admissions counseling.
• Review EPIC for proper documentation and orders.
• Facilitate PAS screening and Level II as needed.
• Assist Social Services team with various tasks.
• Participate in Vulnerable Adult investigations and documentation.

Pay Range: $58,000 - $65,000, based on experience
Schedule: M - F, 8:00am - 5:00pm, occasional evenings and weekends

*This position will oversee both sites, and will also do home visits in Jefferson County*

Objective: Under the supervision of the Marketing Manager, obtains initial information relating to admissions, coordinates intake process, and provides information and referrals to community agencies as appropriate. Promotes the Senior CommUnity Care Program to potential participants and community
agencies.

About the Admissions Coordinator Job:

• Ability to process and complete admissions
• Provide pre-admissions counseling, information and tours
• Review EPIC for proper documentation and orders
• Facilitate PAS Screening and Level II as needed
• Ability to assists Social Services team with various tasks
• Ability to participate in Vulnerable Adult investigations and complete necessary documents/report.
• Education: Bachelor's Degree in humanities/health and human services or equivalent experience with C.N.A. or LPN certification.
• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
• Must have a valid driver’s license and have means of transportation.
• Must clear background check.
• Two (2) years of experience in marketing, with more emphasis on sales, and/or admissions role for a health care provider, or in an information and referral resource role within the senior networks. Comfortable working in a fast pace environment. Excellent customer service!

Preferred Qualifications:

• General knowledge of Medicare, Medicaid, insurance, VA
• Skilled nursing experience
