Responsibilities

• Conduct timely outreach to referred SNAP and TANF participants via phone, text, email, or virtual methods.
• Complete enrollment and intake activities in accordance with program requirements.
• Identify barriers to participation and coordinate referrals to supportive services.
• Document participant interactions, outcomes, and referrals accurately in databases.
• Assist with development of individualized participant service plans with the team.
• Support coordinators in transitioning participants to coaching, workshops, or training opportunities.
