Job summary

Neighborhood Healthcare is seeking an Outreach and Enrollment Counselor in California. You will conduct outreach and in-reach, educate clients, and assist with enrollment in Covered CA, Medi-Cal, and CHIP, plus CalFresh support.

You’ll build community partnerships and help meet grant-driven goals while engaging with diverse populations and coordinating with local health programs. The role requires travel within the community, strong communication skills, and the ability to manage multiple tasks

Qualifications

• High school diploma or GED required.
• Covered California certification within 90 days of employment.
• Experience in government or community program eligibility/screening for Medi-Cal preferred.
• Certified Enrollment Counselor for Covered CA preferred.
• Valid CA driver’s license and proof of auto insurance.
• Bilingual English/Spanish highly preferred.

Responsibilities

• Conduct outreach at schools and community events such as health fairs and local venues.
• Network and develop community relationships and partnerships.
• Track and report monthly and yearly goals per grant contracts.
• Assist in patient retention and new patient growth.
• Educate individuals and families about affordable health insurance options.
• Provide enrollment assistance for Covered CA, Medi-Cal and CHIP as applicable.
• Educate on CalFresh and other community resources.
• Refer applicants to medical homes and other resources.
• Prepare monthly outreach/in-reach activity reports.
