Job summary

Neighborhood Health seeks an Outreach and Enrollment Counselor to educate and assist families in enrolling for Covered CA, Medi-Cal, Medicare, and Cal Fresh. You will conduct outreach at events and partner sites, and support retention and new enrollment efforts.

Responsibilities include presenting health insurance concepts, collecting data for reports, and coordinating with medical homes while working a flexible schedule to support community events.

Responsibilities

• Conducts outreach at schools and community events to educate and enroll in Covered CA, Medi-Cal, Medicare, and Cal Fresh.
• Builds and maintains community relationships and partnerships.
• Tracks and reports outreach and enrollment data for monthly reporting.
• Provides enrollment assistance and education for health insurance programs.
• Guides applicants through the enrollment process and connects them to a medical home.
