DOHC is seeking a detail-oriented coordination specialist to support high-risk urology members. You will interview patients, coordinate with the healthcare team, and assist with referrals to ensure timely access to care.

Responsibilities

• Provides coordination to high-risk Urology members.
• Coordinates certain operations including collaboration of health care team.
• Facilitates effective communication between patient, their representatives and entire DOHC network.
• Assists in obtaining relevant supporting medical documentation and creating, completing, revising and submitting referrals.
• Interviews patients and families to identify potential barriers hindering patient care.
• Organizes, prioritizes and implements daily tasks related to the timely access of member care.
• Document and communicate effectively problem, intervention and plan with EMR.
• Be an ambassador of DOHC and exhibits professionalism in all daily functions.
