Job summary

Clarvida is seeking an Enhanced Community Health Worker (ECHW) in Orange, California to provide culturally responsive, community-based support for individuals with behavioral health and substance use needs enrolled in SMHS/DMC programs.

You will help clients navigate care, access resources, and stay engaged in treatment using trauma-informed, strengths-based approaches; duties include outreach, coordination, and documentation in the EHR system.

Qualifications

• Completion of a Community Health Worker certificate program or equivalent experience aligned with Community Health Worker competencies
• Lived experience reflective of the populations served
• Strong communication and relationship-building skills
• Valid driver's license and transportation if field travel is required
• Ability to complete continuing education requirements

Responsibilities

• Conduct outreach and engagement with individuals facing behavioral health challenges and social needs
• Build trusting relationships using trauma-informed, culturally responsive approaches
• Support re-engagement for clients at risk of disengagement from services
• Provide education on mental health, substance use, wellness, and healthy lifestyles
• Assist with appointment scheduling, referrals, and transportation
• Document services and outcomes in the electronic health record
