Qualifications

• Graduate from an accredited school for health professionals; bachelor’s degree preferred or a certification without bachelor’s such as LVN, MA, or another associated health licensed professional.
• Medi-Cal Peer Support Specialist Certification required or must be obtained within the first 90 days of employment.
• Valid California driver’s license and auto insurance; proof of education required.

Responsibilities

• Provides linkage services for members with physical, dental or mental health care needs.
• Under direction and supervision, may provide health education and community linkage.
• Develops and teaches Wellness related classes and groups.
• Plans, facilitates, and executes annual Health and Resource Fair.
• Maintains HIPAA compliance and professional boundaries with staff and clients.
• Collaborates with community resources to assist in linkage.
