JOB TITLE: Engagement Specialist

EMPLOYMENT TYPE: Full-time, 40 hours/wk

SCHEDULED HOURS: Monday-Friday, 8AM-4:30PM OR 8:30AM-5PM

The Adult Outpatient Services program (AOP) assists adult clients who are recovering from traumatic incidents, substance use, or are looking to manage a chronic mental health disorder through psychiatric & substance abuse diagnostic evaluation and treatment. AOP has a tier-based productivity incentive program, with the vast majority of staff consistently exceeding productivity expectations and obtaining a bonus. Typical days varies as there is the possibility to work with clients individually, in a group setting, or IOP. There is regular weekly supervision provided to all non-licensed staff and biweekly supervision for licensed staff. Nonetheless, supervisors are always available whenever you are working to answer questions and offer clinical assistance when needed.

ABOUT THE POSITION:

To work in tandem with clinical staff to assist clients in accessing care, addressing obstacles to treatment and following up with clients who are having difficulty engaging in care to help them to continue to access the care they need.

By:

  • Providing telephone outreach for individuals and their families to address barriers that keep them from treatment.
  • Working with clinical staff to schedule clients to meet their need.

DUTIES & RESPONSIBILITIES:

  • Establishes rapport and working relationships with clients and clinical staff in order to engage clients in treatment
  • Review client charts to determine eligibility for services based on CHR policy guidelines and physician recommendations.
  • Provide education to clients about resources to assist them in accessing treatment, i.e., transportation alternatives, schedules of providers
  • Coordinate, network and schedule hospital, urgent care, and appropriate internal/external programs within compliance guidelines.
  • Document and track all client hospitalizations in EMR.
  • Assists individuals in making connection to treatment and providing coaching to make it to appointments and to follow up with treatment recommendations.
  • Schedules clients with providers to help them access treatment in a timely fashion. Then follows up with identified high need clients prior to and after appointments.
  • Coaches clients in securing medical benefits in order to pay for treatment,
  • Acts as liaison to transportation providers to enable clients to access treatment services.
  • Coordinates communication between client and clinical staff in regards to no show appointments and schedule changes.
  • Meets all recordkeeping requirements of program and agency standards including monitoring and charting all outreach and engagement attempts.
  • Attends and participates, as appropriate, staff meetings, case conferences, and supervision.
  • Works with assessment center and front desk staff to coordinate access for clients who are high risk or difficult to serve, i.e. individuals being discharged from higher level of care.
  • Provides excellent customer service to all clients and families.
  • Assist staff in promoting a recovery oriented treatment environment.
  • Stays abreast of all agency and clinic policies regarding access, no shows and scheduling.
  • May perform other assignments and duties as program and agency needs require

QUALIFICATIONS:

Education: Training as Medical Assistant preferred.

Experience: Experience in working in fast paced environment with consumers of MH and SA services.

Licensure/Certification/Registration: Certification as a Medical Assistant is preferred

COMPENSATION*:

$18.00/hour min and up commensurateupon experience.

*Actual rates are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
