Upperline Health, Inc. seeks a Nurse Practitioner to deliver endocrinology care across clinic, home, or telehealth settings. You’ll work with physicians and a multidisciplinary team to build complex care plans, adjust therapies, and educate patients on diabetes technology, nutrition, and self-management.

The role emphasizes diabetes/thyroid/metabolic health with a focus on compassionate, detail-oriented care within a growing value-based practice. Regular weekday hours are offered.

Qualificações

• Master’s degree in nursing required.
• Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner required.
• Current State RN & Nurse Practitioner license required.

Responsabilidades

• Develop and implement clinical care plans for adults with endocrine and metabolic conditions.
• Order/interpret diagnostics in clinic, telehealth, or home visits.
• Initiate and titrate diabetes/metabolic therapies.
• Review glucose data and coach patients to improve control.
• Provide diabetes technology education and support (CGM, insulin pump).
• Educate patients on medications, lifestyle, and sick-day rules.
• Coordinate preventive care with the interdisciplinary team.
• Document and communicate care plans with physicians and care teams.
• Participate in weekly team meetings and continuing education.
