Job summary

Vital HealthCare Solutions is hiring a Part-Time Nurse Practitioner (NP) or Physician Assistant (PA) with endocrinology experience to deliver endocrine care in long-term care and skilled nursing facilities.

The role emphasizes diabetes management, thyroid care, and medication management with a focus on geriatric patients. Flexible weekday hours and a collaborative team environment are offered.

Qualifications

• Active Pennsylvania NP or PA license required.
• Experience in endocrinology, diabetes management, or internal medicine preferred.
• Knowledge of insulin therapy, CGM devices, and thyroid medications helpful.

Responsibilities

• Diagnose and manage diabetes mellitus (Type 1 and Type 2).
• Prescribe and adjust insulin and non-insulin diabetes medications.
• Manage and interpret CGM systems.
• Provide thyroid disorder management (hypo/hyperthyroidism).
• Perform patient assessments, treatment planning, and follow-up care.
• Collaborate with interdisciplinary teams in long-term care facilities.
• Maintain accurate EMR documentation.
