Job summary

Catholic Health in Melville, NY is seeking a Nurse Practitioner to join our Endocrinology team. The role focuses on diabetes management and longitudinal care within a collaborative multidisciplinary setting, with progressive autonomy as experience grows.

The NP will work closely with Dr. Md Shajjad Hossain and APPs, handling routine endocrine follow-up, CGM data review, diabetes medication adjustments, and patient education.

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• Current New York State NP license.
• Appropriate national board certification.
• BLS/ACLS required; DEA certification required.

Responsibilities

• Evaluate and manage patients for routine endocrine follow-up.
• Develop expertise in diabetes and other endocrine care.
• Review CGM data and adjust treatments accordingly.
• Adjust diabetes medications and insulin regimens based on findings.
• Educate patients on medications, technology, and disease management.
• Maintain accurate clinical documentation.
