Job summary

Qualifications

• Active Pennsylvania NP or PA license required.
• Endocrinology, diabetes management, or internal medicine experience.
• Geriatric care experience preferred.
• Strong clinical assessment and diagnostic skills.

Responsibilities

• Diagnose and manage diabetes mellitus (Type 1 and Type 2).
• Prescribe and adjust insulin and non-insulin diabetic medications.
• Monitor and interpret CGM data.
• Manage thyroid disorders (hypo-/hyperthyroidism).
• Perform patient assessments, treatment planning, and follow-up.
• Collaborate with interdisciplinary teams in long-term care facilities.
• Maintain accurate EMR documentation.
