Vital HealthCare Solutions in Pennsylvania is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) with endocri­nology experience to provide endocrine care in long-term care and skilled nursing facilities. The role offers flexible, part-time hours with no on-call, no evenings, and no weekends, focusing on geriatric patients in a collaborative team.

Qualifications

• Active and unrestricted Pennsylvania NP or PA license
• Endocrinology, diabetes management, or internal medicine experience
• Knowledge of insulin therapy, CGM devices, and thyroid medications
• Experience in long-term care, skilled nursing, or geriatric care preferred
• Strong clinical assessment and diagnostic skills

Responsabilités

• Diagnose and manage diabetes mellitus (Type 1 and Type 2)
• Prescribe and adjust insulin and non-insulin diabetic medications
• Manage and interpret CGM systems
• Provide thyroid disorder management (hypothyroidism, hyperthyroidism)
• Perform patient assessments, treatment planning, and follow-up care
• Collaborate with interdisciplinary teams in long-term care facilities
• Maintain accurate and timely EMR documentation
