Job summary

Vital HealthCare Solutions seeks a Nurse Practitioner or Physician Assistant with endocrinology experience to serve in long-term care and skilled nursing facilities. This part-time role offers schedule flexibility, clinical autonomy, and a collaborative team focused on geriatric care.

Responsibilities include diabetes and thyroid management, insulin planning with CGM devices, EMR documentation, and patient follow-up, with Monday–Friday hours and no evenings, weekends, or on-call duties.

Qualifications

• Active Pennsylvania NP or PA license is required.
• Endocrinology, diabetes management, or internal medicine experience helpful.
• Experience with long-term care, skilled nursing, or geriatric care preferred.
• Strong clinical assessment and diagnostic skills required.
• Knowledge of insulin therapy, CGM devices, and thyroid medications advantageous.

Responsibilities

• Diagnose and manage diabetes mellitus (Type 1 and Type 2).
• Prescribe and adjust insulin and non-insulin diabetic medications.
• Manage and interpret CGM systems.
• Provide thyroid disorder management (hypo/hyperthyroidism).
• Perform patient assessments, treatment planning, and follow-up care.
• Collaborate with interdisciplinary teams in long-term care facilities.
• Maintain accurate EMR documentation.
