Job summary

Talent With Heart in New York is seeking an Endocrinology Nurse Practitioner for outpatient care in a physician-led, long-standing group with locations across Long Island. The role is 100% outpatient, with flexible hours and options for part-time or full-time employment, Monday–Friday and no on-call requirements.

The NP will diagnose and treat endocrine conditions, develop individualized treatment plans, educate patients, and collaborate with referring physicians and care teams to ensure

Qualifications

• BSN and MSN from an accredited nursing program.
• Licensed NYS RN and NP, BLS, board-certified.
• 8+ years experience as a NP.
• Trained in Endocrinology.
• Strong diagnostic and interpersonal skills.
• Entrepreneurial mindset with ability to grow patient base.

Responsibilities

• Serve as a key clinical provider in delivering high-quality care.
• Diagnose and treat endocrine conditions including diabetes and thyroid disorders.
• Develop and implement individualized treatment plans.
• Refer patients to appropriate specialty services as needed.
• Provide education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough documentation within the EMR system.
• Collaborate with referring physicians, specialists, and care teams for seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff.
• Stay compliant with HIPAA, NYS DOH, CMS, and payer guidelines.
• Contribute to sustainable patient panel aligned with practice growth objectives.
