Job summary

Cabarrus Rowan Community Health Centers is seeking a Referral Coordinator in a clinical setting to manage external referrals. You will coordinate the complete referral process from initiation to receipt of specialty reports and forwarding to the referring provider.

The role requires knowledge of medical terminology, MS Office proficiency, and strong communication skills. English/Spanish bilingual ability is a plus and patient advocacy may be involved as needed.

Qualifications

• Knowledge of medical terminology.
• Proficient with Microsoft Office.
• Excellent verbal and written communication skills.
• Knowledge of local insurance carriers and plans.
• Bilingual in English and Spanish is a plus.

Responsibilities

• Coordinate patient referrals to specialty providers throughout the entire referral process.
• Assemble information concerning patient’s clinical background and referral needs; provide appropriate clinical information to specialist.
• Communicate with specialty office to schedule appointments.
• Contact insurance companies to ensure prior approvals are met.
• Organize and track ordered, scheduled and pending referrals.
• Maintain documentation in the EHR for patient and referral communications.
• Review referral details with patients, including financial obligations.
• Advise patients/providers on referral questions; may advocate with carriers.
• Establish and maintain relationships with service providers.
• Ensure referrals are addressed in a timely manner; prioritize urgent items.
• Work with staff to ensure consult notes are received and shared with the provider.
• Participate in quality improvement activities related to program goals.
