Job summary

Cabarrus-Rowan-Community-Health-Center is seeking a Referral Coordinator to manage the end-to-end referral process for specialty care. You will assemble clinical information, liaise with specialists, coordinate scheduling, and ensure timely completion of referrals with proper documentation.

The role requires strong medical terminology knowledge, proficiency with Microsoft Office, excellent communication, and the ability to navigate insurance requirements. Bilingual English/Spanish is a plus.

Qualifications

• Associates degree in healthcare field required.
• 2 years of related work experience preferred.
• Knowledge of medical terminology.
• Proficient with Microsoft Office.
• Excellent verbal and written communication skills.
• Knowledge of local insurance carriers and plans.
• Bilingual in English and Spanish preferred.

Responsibilities

• Coordinate patient referrals to specialty providers through the full referral process.
• Assemble patient clinical information and refer to guidelines for specialists.
• Communicate with specialty offices to schedule appointments.
• Contact insurance companies to ensure prior approval requirements are met.
• Organize and track ordered, scheduled and pending referrals.
• Maintain documentation in electronic health record for communications.
• Review referral details with patients, including financial obligations.
• Serve as a resource for patients/providers on referrals and advocate when needed.
• Establish relationships with service providers and maintain them.
• Ensure referrals are addressed timely, prioritizing urgent items.
• Work with staff to ensure consult notes are received and directed.
• Contribute to quality improvement initiatives and program goals.
• Support Meaningful Use, PCMH, UDS requirements through documentation.
