Job summary

Ohio Living is seeking a Patient/Staff Service Coordinator to manage staff schedules for home health, home care, and hospice services using the EMR system. You will align physician orders with staff availability and adjust plans as patient needs evolve.

Responsibilities include routing staff, scheduling certification periods, and ensuring accurate billing codes while collaborating with case managers and supervisors to maintain high-quality patient care.

Qualifications

• Two years experience in staff scheduling.
• One year experience in home health, hospice, or medical office management preferred.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.

Responsibilities

• Uses the EMR system to prepare the pre-schedule according to physicians' orders and supervisors' direction for all home health, home care, and hospice visits.
• Routes and assigns field staff in geographic zones to minimize drive time and mileage, and maximize productivity.
• Adjusts schedule according to staff availability and patients' changing needs.
• Schedules the entire certification period at the beginning of the certification period.
• Designates correct billing codes for each visit on the schedule.
• Attends case conferences as requested.
