Job summary

The Recovery Counselor at Trilogy, Inc. will assess clients across personal, medical, emotional, social, and environmental domains to plan treatment via home, community, and office visits.

You will deliver client-centered, strengths-based, and trauma-informed services for severe mental illness with co-occurring substance use disorders using harm-reduction principles. You will perform case management tasks, maintain real-time MAR documentation, provide crisis coverage, and coordinate with care

Qualifications

• Must have one of the listed degree/certification paths in mental health.
• Valid Illinois Driver’s License with daily access to a vehicle and $100,000/$300,000 auto liability insurance.
• Ability to work evenings and weekends as directed.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate recovery plans with client, support system, and care providers.
• Complete daily progress notes and documentation per policy and standards.
• Maintain MAR documentation and coordinate with team nurse/pharmacy for medication accuracy.
• Provide crisis on-call coverage and identify levels of care with referrals as needed.
• Educate clients with psychoeducation, medication training, and monitoring per policy.
• Assist clients with independent living skills and entitlements applications (SSI, SSDI, Medicare, Medicaid, LINK).
• Accompany and transport clients to appointments and provide support for housing, budgeting, and social needs.

Education

Bachelor's degree in counseling and guidance

Bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting

Licensed Practical Nurse under the Nurse Practice Act

Certificate of Psychiatric Rehabilitation from a DHS-approved program

CRSS certification through IAODAPCA

Family Partnership Professional certificate IAODAPCA

Occupational Therapy Assistant license

High school diploma or GED with five years of supervised clinical experience in mental health
