Job summary

The Relief Recovery Counselor at Trilogy, Inc. will assess clients’ personal, medical, emotional, social, and environmental situations to plan treatment across home, community, and office visits. They provide client-centered, trauma-informed services to support recovery from mental illness and co-occurring disorders.

They will deliver at least 75% of services in the community, maintain a shared caseload, and participate in crisis/on-call coverage while collaborating with a multidisciplinary team.

Qualifications

• Bachelor’s degree in counseling or related field or equivalent supervised clinical experience.
• Two years of supervised clinical experience in a mental health setting if degree is in another field.
• LPN license under Illinois Nurse Practice Act or related credential.
• IAODAPCA CRSS or Family Partnership certificates are preferred.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with client, support system, and other care providers.
• Identify and refer to additional services as needed for appropriate levels of care.
• Assist clients in recognizing signs of de-compensation and develop coping strategies.
• Support independent living skills including self-care, housekeeping, nutrition, and shopping.
• Assist with applying for entitlements such as SSI, SSDI, Medicare, and Medicaid.
• Accompany clients to appointments and assist with housing, budgeting, and medication issues.
• Advocate for clients and empower self-advocacy.
• Communicate within a multidisciplinary team for integrated care.
• Perform other related duties as assigned.
