Job summary

The Recovery Counselor at Trilogy, Inc. will assess clients’ biopsychosocial needs across home, community, and office visits, developing individualized recovery plans and linking clients to community services.

The role emphasizes trauma-informed, strengths-based, and harm-reduction approaches to support clients with serious mental illness and co-occurring substance use disorders. Responsibilities include documenting services, providing crisis coverage, helping with entitlements, and coordinating

Qualifications

• Bachelor's degree or equivalent mental health experience as listed.
• Licensed or certified credentials per Illinois requirements.
• Ability to work with a diverse client base in community settings.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation via home, community, and office visits.
• Coordinate individual recovery plans with clients and care providers.
• Complete daily progress notes and maintain MAR and documentation per policy.
• Provide crisis on-call coverage and assist in transitions to higher levels of care.
• Educate clients on entitlements and assist with housing, budgeting, and independent living skills.
• Advocate for clients and coordinate with team to ensure coordinated care.

Education

Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field

Bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting

Licensed Practical Nurse under the Nurse Practice Act [225 ILCS 65]

Certificate of Psychiatric Rehabilitation from a DHS-approved program, a high school diploma/GED, and two years’ experience of experience in a mental health setting

CRSS certification through IAODAPCA

Family Partnership Professional certificate from and in good standing with IAODAPCA

Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with one-year experience in a mental health setting

High school diploma or GED and five years of supervised clinical experience in a mental health setting
