Job summary

Forward Service Corporation is seeking an FSET Case Manager in Jefferson, WI to support individuals pursuing employment and self-sufficiency. You will coach, advocate, and connect participants with resources, education, and job opportunities to help them reach personal and professional goals.

Ideal candidates bring an Associate’s degree or equivalent, 2+ years in case management or related fields, and strong communication, organizational, and problem-solving skills.

Qualifications

• Associate's degree or equivalent combination of education and experience.
• Two or more years of experience in case management, workforce development, social services, employment services, or a related field.
• Excellent communication, organizational, and problem-solving skills.
• Ability to build rapport with diverse populations.
• A positive attitude, adaptability, and commitment to participant success.

Responsibilities

• Provide strengths-based coaching and support that empowers individuals to succeed.
• Assess participant needs and develop personalized employment plans.
• Connect clients with job training, education, volunteer opportunities, and community resources.
• Facilitate engaging job readiness workshops and employment search activities.
• Monitor progress, celebrate milestones, and help participants stay motivated along the way.
• Make a meaningful impact in the community through participant success.
