Open Sky Community Services in Massachusetts is seeking Outreach Counselors to deliver evidence-based interventions for adults with mental health needs. You will work under supervision to help individuals transition toward independent living and meaningful community participation.

The role emphasizes employment support, education coordination, and collaboration with families and agencies. Open Sky values trauma-informed care and offers strong supervision and growth opportunities.

Qualifications

• High School Diploma, GED or equivalent, required.
• Reliability, willingness to learn, and being open to new opportunities are essential.
• Valid driver’s license, reliable and insured vehicle, and acceptable driving record are required.

Responsabilités

• Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
• Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
• Provide side-by-side support and coaching to help individuals socialize.
• Conduct both 1:1 and group activities geared toward increasing Persons' readiness to engage in rehabilitation.
• Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.
• Participate in the On-Call rotation.
