Job summary

Angels Care Home Health is seeking an Occupational Therapist to provide clinical assessment, plan of care development, and ongoing documentation for patients in the St. Louis area.

The role supports a patient-centered home health program with emphasis on safety, infection control, and effective teamwork. Qualified candidates will hold an OT license in good standing, have at least two years of practical experience, and possess current CPR and Hepatitis profile.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• CPR and Hepatitis profile current.

Responsibilities

• Perform clinical assessment and identify patient needs and appropriateness of patient care.
• Document, prepare, and complete a Plan of Care, progress notes, and other clinical record documentation on an ongoing basis.
• Revise the Plan of Care based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in coordinating/performing patient care.
• Accept clinical assignments that are within education and competence to care for patients.
• Use interpersonal relations and communication skills to coordinate with the care team for desirable outcomes.
• Assess physical, social, and cultural components of the patient’s occupational environment.
