Job summary

Angels Care Home Health, a Medicare-certified home health provider, seeks an Occupational Therapist to perform clinical assessments, develop and revise a Plan of Care, and coordinate patient care in the home setting. Based in Granby, MO, you will work with physicians and care teams to deliver compassionate, quality services.

The role requires an active OT license, at least two years of experience, current CPR and Hepatitis status, and knowledge of durable medical equipment.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment.
• Identify patient needs and appropriateness of patient care.
• Document, prepare, and complete a Plan of Care, progress notes, and other clinical record documentation on an ongoing basis.
• Revise the Plan of Care based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in coordinating/performing patient care.
• Assess physical, social, and cultural components of the patient’s occupational environment.
