Community Health and Wellness of Greater Torrington, Inc. is seeking a Full-time Maternal Home Visit Community Health Worker to support pregnant individuals, new mothers, infants, and families through home-based education, care coordination, and connections to community resources.

The CHW promotes healthy pregnancies, child development, and family well-being by coordinating with nurses and social services, documenting visits, and traveling to client homes.

Qualifications

  • High school diploma or GED required; associate's degree preferred.
  • Certification as a Community Health Worker preferred or willingness to obtain certification.
  • Experience with maternal/infant populations in community or healthcare settings preferred.
  • Knowledge of maternal and child health, resources, and health education principles.
  • Strong communication, interpersonal, and cultural competency skills.
  • Ability to build trusting relationships with diverse populations.
  • Proficiency with EHRs and Microsoft Office.
  • Valid driver's license and willingness to travel for home visits.

Responsibilities

  • Conduct scheduled home visits with pregnant individuals, postpartum mothers, infants, and families.
  • Provide education on prenatal care, postpartum recovery, infant care, breastfeeding, nutrition, safe sleep practices, and child development.
  • Assess family needs and identify barriers to healthcare, housing, food security, transportation, and other social determinants of health.
  • Connect families with healthcare providers, social services, community programs, and support resources.
  • Assist clients in scheduling and attending prenatal, postpartum, pediatric, and specialty medical appointments.
  • Promote healthy lifestyle behaviors and encourage adherence to healthcare recommendations.
  • Provide emotional support, advocacy, and culturally responsive care to families.
  • Maintain accurate documentation of home visits, assessments, referrals, and client progress in accordance with agency policies.
  • Collaborate with nurses, social workers, healthcare providers, and community organizations to coordinate comprehensive care.
  • Participate in outreach activities, health education events, and community engagement initiatives.
  • Maintain client confidentiality and comply with HIPAA and organizational standards.

We are currently recruiting for Full time Maternal Home Visit Community Health Workers - 5 year grant funded

Job Summary:
The Family Bridge Maternal Home Visit Community Health Worker (CHW) supports pregnant individuals, new mothers, infants, and families by providing home-based education, care coordination, and connections to community resources. The CHW serves as a trusted liaison between families and healthcare providers, promoting healthy pregnancies, positive birth outcomes, child development, and family well-being.

Key Responsibilities:

  • Valid driver's license, reliable transportation, and willingness to travel for home visits.
Preferred Skills:
  • Bilingual abilities (English/Spanish or other languages) are a plus.
  • Experience with home visiting programs or case management.
  • Knowledge of trauma-informed care, motivational interviewing, and family-centered care approaches.
  • CPR and First Aid certification preferred.
