Job summary

CenterWell Home Health is hiring a Speech-Language Pathologist to evaluate and provide SLP services in patients' homes or facilities. You will participate in care planning, assess speech, language, voice and swallowing disorders, and contribute to treatment plans and coordination.

The role requires ASHA CCC certification, a current state license, CPR certification, and a valid driver's license with reliable transportation. Home health experience is preferred.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology (ASHA) required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted state license and CPR certification.

Responsibilities

• Evaluate, direct and provide speech/language pathology services in home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; perform audiologic screening.
