Job summary

CenterWell Home Health in Plano, TX is seeking a Speech Language Pathologist to evaluate and provide therapy to patients in the home setting, reporting to the Clinical Manager. You will develop and review plans of treatment, assess speech, language, voice, and swallowing disorders, document results, and coordinate care with patients, families and healthcare staff.

Required: at least 6 months of SLP experience, active license, CPR, and valid driver's license with reliable transportation; home

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening.
• Observe, record and report changes in the patient's condition and response to treatment to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Counsel and instruct patients, families and healthcare staff; maintain documentation and submit reports as required.
• Participate in care coordination and discharge planning.
