Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to provide in-home evaluation and treatment for patients with speech, language, voice, and swallowing disorders. You will participate in care planning and document progress, delivering services with professionalism and confidentiality.

The role requires a current license and CPR certification, with at least six months of experience. A valid driver's license and reliable transportation are required to service patients in home

Qualifications

• Minimum six months experience as a speech therapist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.
• Preferred: Home Health experience.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiology screening.
• Observe, record and report changes in the patient's condition and response to treatment.
• Provide instruction and training to patients in alternative communication systems when appropriate.
• Counsel and instruct patients, families and healthcare staff.
• Submit written reports of evaluations, visits, care plans and progress reports.
• Participate in care coordination activities and discharge planning.
• Maintain confidentiality and follow company policies.
