The role emphasizes personalized, evidence-based care, collaboration with a multidisciplinary team, and documentation of progress. Onsite, with flexible scheduling and dedicated team support to promote patient wellness.

Qualifications

• Active Texas PTA license.
• Current CPR certification.
• Professional liability insurance coverage.
• Experience in home health or rehabilitation settings preferred.
• Collaborate effectively within a multidisciplinary care team.
• Clinical experience as a PTA.
• Strong interpersonal skills and adaptability in care approaches.
• Up-to-date knowledge of current treatment techniques.
• Professional and compassionate when educating and engaging with patients.

Responsibilities

• Deliver home-based physical therapy services under supervision of a licensed PT.
• Identify patient goals and implement interventions to meet mobility needs.
• Provide evidence-based, cost-effective treatments to improve movement.
• Support alternatives to surgery and reduce medication reliance through therapy.
• Develop and carry out individualized care plans with proven techniques.
• Create wellness and fitness programs tailored to each patient.
• Educate and motivate patients to participate in rehabilitation.
• Promote health by improving strength, flexibility, coordination and balance.
• Collaborate with other healthcare professionals to ensure coordinated care.
• Monitor progress, evaluate outcomes, and communicate updates to the team.
• Accurately document all services and maintain complete records.
