Job summary

CenterWell Home Health is seeking a licensed Physical Therapy Assistant to provide in-home physical therapy under the supervision of a Physical Therapist. You will administer treatment plans, perform therapeutic exercises and modalities, document patient records, and coordinate with the team to ensure high-quality care.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license with reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric changes (physical, emotional, social, biological).
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, onsite in patients' homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapies, therapeutic exercises, modalities to patients.
• Document client records and submit clinical documentation on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment/supply/staff changes.
• Apply infection control and universal precautions in patient care.
