Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes under a Physical Therapist. You will help patients regain strength, mobility, and independence with therapy focused on functional improvement.

Responsibilities include administering treatment plans, documenting progress, coordinating with the care team, and ensuring safety via infection control and safety practices.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Geriatric population physical, emotional and social changes knowledge.
• Read, write, and speak fluent English.

Responsibilities

• Administer the PT treatment plan to home patients under supervision.
• Deliver non-complex manual therapy, massage, modalities as prescribed.
• Document client progress and submit records weekly.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment issues or staffing changes.
• Apply infection control and universal precautions in care delivery.
